This Privacy Policy describes how your personal information is collected, used, and shared when you visit https://www.floridacommunityhealth.org ("FloridaCommunityHealth.org"), this site, or when communicating with us through SMS messages (collectively "Texts" or "texting"). This policy is in addition to the Privacy Practices for protected health information ("PHI") detailed as part of our Patient Notices, which is available on the Forms & Downloads tab on this website.

Text Messaging

If you provide a mobile telephone number and consent to receive Texts from us, we may use Texts to communicate with you about services, payments, account matters, or other services you request. How We Use Your Information

SFCHC does not intentionally store payment card numbers on its own website server. Payment transactions are processed by our third-party payment processor. Other limited website activity information may be collected by our hosting, analytics, payment, or communications vendors as described in this Policy.

Personal information

We use the Device Information that we collect (in particular, your IP address) to help us screen for potential risk and fraud. We also use Google Analytics to help us understand how visitors use FloridaCommunityHealth.org and to gauge the success of our marketing campaigns.

Payment information

We use the Card Connect Merchant Services Partner Program to process credit card payments you make to SFCHC. The Payment Information we collect is passed to Card Connect. Do Not Track

Note that we do not alter FloridaCommunityHealth.org's data collection and use practices when we see a Do Not Track signal from your browser.

Changes

We may amend this Privacy Policy from time to time in order to reflect changes to our practices or for other operational, legal, or regulatory reasons. This Privacy Policy was last updated on July 20, 2026.

This Privacy Policy shall be governed by and construed in accordance with the laws of the State of Florida.
