Sliding-Scale Fee Schedule
South Florida Community Health Centers is committed to helping local residents with financial need. We offer reduced fees to eligible patients without insurance. The amount you pay for an office visit depends on your household income, relative to federal poverty guidelines.
To qualify for these rates, you must provide proof of household income, such as recent pay stubs and prior-year tax returns, and complete a confidential Affidavit of Income. You can download the affidavit from the Forms & Downloads menu.

Does SFCHC accept walk-ins?
We welcome walk-in patients and accommodate them on a first-come, first-served basis. However, we strongly recommend calling (954) 603-1000 before your visit so we can confirm that we have any vaccines, testing supplies, or other materials you may need.
Do you treat children?
SFCHC primarily treats adults, but we offer school physicals for patients age 14 and up.
Who are your providers?
Care at SFCHC is provided by a nurse practitioner with advanced training and skills.
When do you collect fees? Are there payment plans?
Payment is expected at the time of service, either in cash or when you provide evidence of insurance through a government program (such as Medicare, Medicaid, or Tricare) or commercial insurance. For cash payers, the full amount is due when service is rendered. For others, remaining plan deductibles, office copays, and estimated co-insurance amount must be paid.
Do you write prescriptions?
Yes, our nurse practitioner can write prescriptions when medically appropriate, typically for antibiotics (like a "Z-Pak"), antifungals, antihypertensibles for high blood pressure, statins for cholesterol, and common antidepressants.
We do not write highly specialized prescriptions—such as long-term opioid pain management regimens, chemotherapy agents, or complex psychiatric drugs—requests for these prescriptions will be referred to board-certified specialists.
Will you write letters for patients?
We write letters—including Letters of Medical Necessity (LMN), Fitness-for-Duty or Return-to-Work Notes, letters verifying the need for an Emotional Support Animal (ESA), work or school accommodations under the Americans with Disabilities Act (ADA), requests for specific ADA-compliant housing accommodations, requests for leaves under the Family and Medical Leave Act (FMLA)—and complete associated forms when necessary.
We will provide such letters at the discretion of our nurse and Medical Director. We may ask for an office visit before agreeing to write letters or prepare forms, and we may charge a preparation fee.
